The Transitions Team focuses on adult clients who are severely mentally ill and are unable to successfully reside outside an institutional setting. The team works closely with API, Department of Corrections and the Mental Health Court in order to help clients be successful when they are transitioning back into the community. The team focuses on teaching skills and basic pro-social behaviors to clients in order to function positively in our community. The team has daily clinical team meetings in order to review every client and near daily contact with each client.

The team holds many groups to help consumers re-enter our community. The offerings rotate based on client need, but include over time:
- Moral Reconation Therapy: this is a group treatment strategy that seeks to decrease “institutional thinking” by increasing moral reasoning.
- Wise Ways: This group teaches healthy skills on how to live a healthy and happy life with a mental illness.

- Rotation of Co-occurring substance and mental illness, including Mentally Ill, Chemically Addicted Adults (MICAA), Life Balance, and Integrated Dual Diagnosis Treatment (IDDT)
